Fees, Payments, and Insurance

Our fees are based on the highly specialized care we provide.  All deductibles and co-payments are expected at the time of service.  The amount collected is an estimate of the amount due for services rendered.  You may still receive a bill from the practice depending on the exact payment made by your insurance company.  Payment schedules are arranged for global pregnancy and surgery patients. No personal checks accepted.  Most major credit cards are welcomed for your convenience.  Fees are charged for the following services:

  • Consultation and office visits according to the complexity of your problems.
  • If a procedure is performed, there is a professional fee for performing the procedure.
  • If blood work or other lab tests (including pap) are performed there will be a separate processing fee charged by LabCorp in addition to the doctor's fee.
  • If the procedure is done at the hospital, there will be a facility fee charged by the hospital in addition to the doctor's fee.

 

We know questions can arise on insurance matters and these may be discussed directly with our insurance office.  Our toll free number to the insurance claims office is 1-800-659-3171.

 

Telephone Calls

Please know that we are interested in hearing of any questions pertaining to your medical care that you may have. Dr. Graham and his professional staff of experienced Registered Nurses will personally respond to your concerns in a timely manner.  Because we are committed to providing quality care, we would prefer you schedule a brief appointment to better serve your needs whenever possible.  If your case is emergent, or you are in any way in doubt, notify our office immediately so that prompt care can be provided.  After office hours, calls are received by the answering service, who will forward all messages to Dr. Graham or the provider on call.

 

Office Hours

Office hours are by appointment only.  Appointments are scheduled by phone during regular business hours.  We are open Monday through Thursday from 9:00 a.m. to 4:00 p.m. and Fridays from 9:00 a.m. to 1:00 p.m.  We do respect your time and would appreciate your calling our office 48 hours ahead of time if you need to cancel for any reason.  Our office uses Electronic Medical Records (EMR).  Please access the patient portal via the internet 24-48 hours prior to your appointment to complete or update your information.  Please bring with you to to your appointment any regular medications in their containers.

 

In the interest of the comfort of all our patients, please make every attempt to have child care arrangements prior to your scheduled appointment.

 

We ask that you please complete all food and beverages prior to arriving at your appointment.

 

Prescriptions

Prescriptions may be requested by phone. Have the prescription name available along with the pharmacy phone number.  These will be called in within 24-48 hours. We must have a record of your visit within the last 12 months at our office in order to call in your prescription.  Controlled substances will not be dispensed by phone.  We do not call in prescription for "self treatment" of problems.  Our office will gladly accommodate your request after a brief examination.  Same day appointments are available for established patients of the practice, when called in by 11 a.m.

 

Lab Results

Wide range of in-office lab work offered:

Our office uses Laboratory Corporation of America (LabCorp) to process samples and specimens.  If your insurance company does not cover services provided by LabCorp, kindly advise our office staff prior to booking your appointment.  Lab and biopsy results take at least 7 to 10 working days to be delivered.  Dr. Graham promptly reviews the results.

  • Pap: Patients may conveniently access their pap results by calling the LabCorp test information line directly, toll free at 1-888-567-8723.  Please allow 7-10 days for processing prior to inquiry.
  • Biopsy: All biopsy reports are disclosed to patients at the scheduled follow up visit.
  • Blood Work: Patients are notified of abnormal values only.
  • Mammogram: Mammogram reports are provided through the diagnostic center.  If the mammogram is in any way abnormal you will also be contacted by this office.

To respect patient confidentiality and to maintain accuracy of reporting, test results are not released to callers by phone.

 

Medical Records

Medical records are released only with the patients written consent.  There is a nominal fee for duplicating medical records.  Records are mailed or faxed within 2-3 working days.
